- $25 setup fee, one time
- 1 free visit per year per member
- $20 per visit, regardless of services; unlimited visits
- Includes: most in house labs, COVID tests (limit 2/yr), xray, laceration repair or most simple procedures, Discount for offsite labs, exclusive discounts, early notification of botox specials

This is NOT medical insurance and cannot be applied to insurance deductibles. This program is designed to be a cash-based direct care program that can offer discounted visits and services for frequent users or uninsured patients.

- 100 units a year (@$11.50/unit) – $95.83/month
- 150 units a year (@11/unit) – $137.5/month
- 200 units a year (@$10.50/unit) – $175/month
- Additional units needed will be at rate paid for membership
- You can pay full price for 1 year at $10.50/unit, with any additional units $10.50/unit on day of visit (minimum 100 units)
- $99 annual to get access to $11/unit pricing
- Minimum 1 year commitment
